Construction worker dies from fall

A construction worker who fell while working on the roof of a two-story building at Randolph AFB last week has died of his injuries.

Pedro Vazquez, 48, who had done roofing his entire working life, slipped while trying to unfasten a rope attached to a harness, said his uncle, Manuel Vazquez, 43.

The accident happened on June 28 around 8:30 a.m., an hour into Vazquez’s second day on the job at that site, his uncle said.

He was transported to University Hospital, where he died Thursday.

The U.S. Department of Labor’s Occupational Safety and Health Administration will review the record of Herzing Roofing LLC and determine whether equipment failure or employee negligence was involved, said Elizabeth Todd, a regional OSHA spokeswoman based in Dallas.

The probe can take up to six months, she said. A search on an OSHA database did not show Herzing Roofing having any violations in the past three years.

Vasquez, also a construction worker, said he would like to see videos orienting workers on safely using harnesses. His nephew, who lived in San Antonio for more than 25 years, was buried in Mexico on Tuesday, he said.

“Pedro liked his work and he did it well,” his uncle said in Spanish. “He always took precautions. So it surprises me that it happened to him.”

Man loses both legs in train accident

A man lost both legs when he jumped off a moving train early Wednesday, police said.

Investigators were summoned to Probandt Street and Lone Star Boulevard at 3:30 a.m. , where two men who had been on the northbound freight train jumped off. The first man cleared the train but his companion, a man in his 40s, may have slipped as he jumped, partially falling on the tracks, police reported.

One of his legs was severed below the knee, and the other sustained massive blood loss and trauma and had to be amputated, authorities said.

He was in undetermined condition at Brooke Army Medical Center.

Man killed in Northwest Side crash

A man was killed when his car veered off the road and crashed Tuesday morning, authorities said.

The Bexar County Medical Examiner’s office identified the victim as Richard Ybarra, 19.

The crash occurred around 4 a.m. when Ybarra’s green 2000 Honda Civic drifted off the road and hit a utility pole in the 12000 block of Galm Road, said Dale Bennett, spokesman for the Bexar County Sheriff’s Office.

Ybarra was dead when officers arrived, Bennett said.

Officials did not know what caused Ybarra to crash. No one else was injured, Bennett said.

Elderly man killed in hit-and-run

An elderly man was killed in a hit-and-run Monday night at an H-E-B parking lot on Marbach Road, police said.

Charles Simpson, 70, was on a crosswalk approaching the store when a white Chevy hit him and sped off, police said. The driver was waiting in a handicap parking spot at the store’s entrance for a woman who was being detained for shoplifting, police said.

When told to move from the parking spot, he became angry, accelerated and crashed directly into the man, police said. He did not slow down or stop to check on the man, police said.

The man was taken to University Hospital where he died from his injuries.

Police are still looking for the driver.

Family suffers second fatality

The president of a local motorcycle club was killed in a crash with a horse trailer in West Bexar County on Saturday night, almost a year after he lost his wife in another motorcycle crash.

Vikk Villarreal, 41, died at the scene of the crash on Potranco Road near Sundance Crest, officials said. He was the president of the Sentinels Motorcycle Club; his wife, Jessica Villarreal, died on her 37th birthday last July 26. The couple had two children.

“We’re still in a state of shock,” said David “Wolf” Laughlin, the Sentinels’ sergeant-at-arms. “We’ve all noticed the timing, and the general consensus is Baby J (Jessica Villarreal) wanted him back.”

Sheriff’s Sgt. E.M. Conger said Villarreal was riding west on Potranco around 9:30 p.m. with two other motorcyclists when he stopped to make a U-turn. Villarreal was the lead rider, Laughlin said, and the trio were headed to get dinner.

The motorcycle behind Villarreal broke down, causing the two other riders to stop, Laughlin said. Villarreal realized the two weren’t behind him and turned around to look for them. He passed them going east and then pulled over to the shoulder to turn again and ride west on Potranco to meet them, Laughlin said.

A Dodge truck pulling a horse trailer was eastbound on Potranco, Conger said, and Villarreal’s red 2008 bike became entangled in the trailer’s wheels.

Conger said criminal charges are not expected to be filed in connection with the crash.

Laughlin said Villarreal’s riding habits hadn’t changed much in the year since he lost his wife. He always wore a helmet, Laughlin said, and stayed committed to the Sentinels.

“He was still always wanting to ride,” he said. “He loved bikes.”

Jessica Villarreal was riding to Medina Lake last July when she lost control of her motorcycle on FM 471. She was hospitalized and resuscitated several times before Vikk Villarreal told doctors to let her go if her heart stopped again.

The couple met as teens and stayed close for decades. They wed in 1998 and later formed a heavy metal band, One of Sixx. Jessica Villarreal sang backup to her husband’s vocals.

As the wife of the Sentinels’ president, she bore the title of the club’s “First Lady.”

Relatives likely will care for the Villarreal’s two orphaned children, Stephen Moreno, 17, and Isiscianna Villarreal, 11. Funeral arrangements are pending.

One killed two injured in New Braunfels crash

A motorist was killed and his two passengers were injured in a single-vehicle rollover in New Braunfels early Sunday, according to officials.

The driver, 18, was pronounced dead at the scene of the crash, which took place around 5:50 a.m. in the 2500 block of Texas 46 South, according to a New Braunfels Police Department news release.

Officials have not yet released the name of the motorist, said to be from New Braunfels. His two passengers, both 21-year-old men from Seguin, were flown to University Hospital. Police said one is listed in stable condition and the other is in fair condition.

The news release states the driver was southbound on Texas 46 when he veered across the northbound lanes and drove into a drainage culvert. The vehicle then struck an embankment, flew into the air, landed upside-down and slid on its roof until coming to a stop in the grassy shoulder, police said.

According to officials, one of the passengers crawled out of the back seat. The other passenger and the driver were trapped in the vehicle, police said, and had to be cut out.

No other information was immediately available, including whether police suspect alcohol or speed played a role in the fatal collision.

Dad forced boy to drive

An Austin man is facing several charges, including felony injury to a child, after police say he forced his 14 year old son to drive while he beat the boy, according to an arrest affidavit.

The May 19 incident began in Southeast Austin after Mohammad Sherzaman, 51, left the car sales lot where he works in the 1600 block of South Pleasant Valley Road, the affidavit said.

The 14 year old and his 5 year old brother left in Sherzaman’s pickup about 11 p.m. after his father had drank about 12 beers and smoked marijuana at the lot, the affidavit said. Sherzaman told the 14 year old to drive to a home where the boys were to be dropped off, the affidavit said.

Sherzaman hit the teen in the head as the boy drove north at about 55 mph along Interstate 35, the affidavit said. The boy pulled over in a store parking lot in North Austin and said he would no longer drive, the affidavit said. Sherzaman began driving but continued hitting the boy, the affidavit said. The boy showed up at the home with visible injuries, and a relative called 911, the affidavit said.

Sherzaman was booked into the Travis County Jail on Thursday and was being held on $95,000 bail on four charges, the affidavit said.

Bus crash shuts down I-35 lanes

A charter bus, bound for Dallas and Des Moines, rolled over on Interstate 35 between New Braunfels and San Marcos, injuring 11 passengers and blocking off the northbound lanes for hours early Friday.

Three passengers with life-threatening injuries were airlifted to University Hospital in San Antonio, said Hays County sheriff’s Lt. Dennis Gutierrez. Eight more were taken by ambulance to hospitals in Kyle, San Marcos and Austin, said San Marcos Police Commander Penny Dunn

The highway was re-opened after the bus was cleared off the road as rush hour hit.

The accident occurred at 4 a.m. on the Hays-Comal county line near Posey Road, just inside the city limits, Dunn said. The bus, with a capacity of 72 passengers, was only 20 passengers at the time, she said.

Neither Dunn nor Gutierrez were able to identify the three seriously injured victims, saying that police officers, deputies and state troopers were still on the scene gathering information. A University Hospital spokeswoman confirmed that two men and a woman from the accident were listed in stable condition, but offered no other details.

The bus is owned by Mares Tours, a Dallas-based bus line. The firm couldn’t be reached for comment, but a company spokesperson told the Associated Press they knew nothing about the accident.

Motorcyclist airlifted after colliding with SUV

A motorcyclist was rushed to the hospital by helicopter Wednesday night after he crashed into an SUV on the Northeast Side and suffered life-threatening injuries.

San Antonio police said the 48-year-old man was traveling north on Judson Road when he rammed his Yamaha V Star into a Jeep Commander pulling out of Sun Country Street.

Another vehicle had obstructed the SUV driver’s view, and neither motorist could have avoided the crash, which occurred shortly after 8 p.m., traffic investigators said. No charges are expected to be filed.

The motorcyclist was wearing a helmet, police said. He remained in critical condition at University Hospital late Wednesday.
